SCCE & HCCA Speaker Form
Presentation Requirements. Speaker agrees to present as a speaker at the conference listed above, and agrees to the following SCCE/HCCA requirements:
- Handouts and presentation materials (which must include a PowerPoint presentation all but panel discussions) must be sent to SCCE/HCCA's designated conference planner by the due date given.
- Handouts and presentation materials must be free of promotional material (including, but not limited to, price and product listings, surveys, and vehicles for collecting participant information).
- No changes may be made to handouts and presentation materials once submitted to SCCE/HCCA. If changes are required, Speaker must obtain advanced approval from SCCE/HCCA.
- Speaker shall participate, as necessary, in conference calls with co-presenters, which shall be facilitated by SCCE/HCCA, for purposes of preparing for and organizing presentations.
- Speaker shall not acquire any rights through this agreement to utilize the SCCE/HCCA name or service mark in connection with the Speaker's professional or personal work.
- Speaker acknowledges that SCCE/HCCA does not restrict attendance at the conference, and that attendees, members of the media and/or SCCE/HCCA staff may comment on or quote from the content of my presentation which may appear in print, online, or any other form of media.
- Additional presentation guidelines
Right to Record Presentation and Use Materials, Name and Likeness. Speaker hereby grants SCCE/HCCA a perpetual, royalty-free license to record, use, sell, reproduce and distribute Speaker's presentation (including the video and/or audio recording of the presentation and all handouts and PowerPoint presentations). Speaker further grants permission to SCCE/HCCA to make the recording of Speaker's presentation, handouts and related material available online through the SCCE/HCCA website. SCCE/HCCA may photograph Speaker during the conference and such photographs may be reproduced or used by SCCE/HCCA and may appear in print, online or in other forms of media. Speaker authorizes SCCE/HCCA to record Speaker's session and use his/her name, likeness, photograph, and biographical data in connection with any aspect of the conference, including the promotion of the conference and the promotion of SCCE/HCCA. Speaker's Representations and Warranties. Speaker represents and warrants that the presentation, handouts, and related materials are factually accurate, contain no matter libelous or otherwise unlawful, and contain no material that is unlawful or improper to publicly disclose without consent, such as Protected Health Information as defined under HIPAA. Speaker further warrants that such presentation, handouts, and related materials are either
- the Speaker's own original work;
- available for use without permission because they are in a public domain; or
- when materials utilized in the presentation are not the Speaker's own original work or in a public domain, they are either:
- presented with the permission of the originator/owner of the material, or
- are believed to be with in the "fair use" exemption of copyright law which includes use for purposes of criticism, news reporting, commentary, teaching, and research.
Speaker will indemnify, defend, and hold SCCE/HCCA harmless from any claim, demand, loss, liability, damage or expense arising from Speaker's breach of the above warranty and/or presentation of the session(s).
Cancellation. SCCE/HCCA reserves the right to cancel any session, change the length or size of any session, or change the place of any session at its discretion at any time.
Expense Reimbursement. Speaker is entitled to expense reimbursement pursuant to the SCCE/HCCA Speaker Reimbursement Policy. Speaker must meet the eligibility requirements stated in the policy in order to be reimbursed expenses for this speaking engagement
Terms and Conditions. By accepting the invitation to speak at this event, you agree to the full event Terms and Conditions, including the use of your information that may be shared with conference exhibitors, attendees, speakers, affiliates, and partners for promotional and/or networking purposes. For Virtual Events: By registering for this event, you are also confirming that you understand that this event registration form allows only the registered attendee to stream, participate, and earn CEUs for the event. If a second person would like to join, they must register themselves and have their own unique login to participate. No audio or video recording by attendees is allowed.
For In-Person Events: By registering for this event you are also confirming that you have read and agree to the Personal Accountability Commitment, the Assumption of Risk and Liability Waiver.
Personal Accountability Commitment. Any public space where other people are present holds an inherent risk of exposure to COVID-19 and other communicable diseases. I will take necessary precautions while at the event including, but not limited to personal hygiene and hand sanitization, adherence to pathway signage and self-monitoring. You should not attend an SCCE/HCCA event if you are experiencing symptoms associated with the flu or COVID-19. You also should not attend if you have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for your treatment.
Assumption of Risk: By submitting this registration, I acknowledge the contagious nature of COVID-19 and other communicable diseases, and voluntarily assume the risk that I may be exposed to or infected by COVID-19 or other communicable disease by attending this SCCE/HCCA event, and the consequences of such exposure. It is my choice to participate in this event, knowing that attending this event may increase the risk of becoming exposed to and infected by COVID-19 or other communicable disease. I voluntarily agree to assume the risk of contracting COVID-19 or other communicable disease, and I accept sole responsibility for any injury or illness to myself or others.
Liability Waiver: In consideration of being permitted to participate in the SCCE/HCCA event, I hereby waive, release from liability, assume all risks, and covenant not to sue SCCE & HCCA or its officers, board members, employees, agents, and representatives (the "SCCE & HCCA Parties") for any expense, loss, damage, personal injury (including loss of life, disability, or serious harm), property damage or theft, negligence, or actions (each, a "Loss") resulting from or arising in connection with my travel to, attendance at, or participation in the SCCE/HCCA event and any related activities unless said Loss is caused by the sole, gross negligence of SCCE/HCCA. I further hereby release, agree not to sue, discharge, and hold harmless SCCE & HCCA, its officers, board members, employees, agents, and representatives, from all Losses relating to COVID-19 or other communicable diseases.
I understand and agree that this release includes any and all claims based on the actions, omissions, or negligence of SCCE & HCCA, its officers, employees, agents, or representatives.
This assumption of risk and waiver applies even if the undersigned asserts that SCCE & HCCA was at fault for not taking greater precautions to manage exposure or infection from COVID-19 and other communicable diseases. I agree that this waiver and release shall bind me and my personal representatives, shall be enforceable to the fullest and broadest extent of the law, and, if any portion is held invalid, the remainder should continue in full legal force and effect.
SCCE & HCCA considers the health and safety of all those at in-person programs our top priority. SCCE & HCCA are committed to providing a safe and healthy environment for all our in-person program participants and staff. Although participants should recognize that there is risk involved in attending, SCCE & HCCA will be following preventative measures to reduce the potential spread of the COVID-19 virus and other communicable disease at its in-person event in accordance with guidance provided by CDC, and/or local or state government agencies, and its partnering hotels. SCCE & HCCA staff will be trained, in the knowledge necessary to monitor, enforce, and handle compliance with safety protocols and procedures.
